Countryside realized that America's current economic climate was difficult this holiday season and we wanted to do our part to ensure everyone enjoyed the opportunity to win a heavenly holiday shopping spree! The donation of the shopping spree was just a small way of saying "thanks" to you for shopping in Countryside. Countryside is an excellent place to get your shopping done, as we are home to a wealth of national retail outlets including Best Buy, Home Depot and Marshalls as well as 13 auto/cycle brands and 11 car dealerships. As one of Chicago's western suburbs, we offer a convenient and accessible location just off I-55 and I-294. Mayor Robert Conrad stated his excitement about Countryside's 12-Days of Christmas Extravaganza: "Countryside is the ideal place to shop close to home, boost the local economy and put yourself in a position to win a fantastic shopping spree," said Mayor Conrad. "I look forward to seeing a very broad smile on the face of whoever ends up winning the shopping spree!" Follow the steps below for your chance to win the $5,000.00 shopping spree at your favorite Countryside retailer. Remember, your entry is due by January 31, 2009! 1. Have spent at least $100 or more at a single Countryside retail store during the 12 days of the contest (see below for the official rules & regulations). 2. Fill out the contest participation form, either on-site at the store or print it here on the Web site 3. CITY OF COUNTRYSIDE 12 DAYS OF CHRISTMAS EXTRAVAGANZA OFFICIAL RULES MINIMUM ONE HUNDRED DOLLAR ($100.00) PURCHASE MADE BETWEEN 12:01 AM CT 12/13/08 AND AT 11:59 PM CT 12/24/08 FROM ANY ONE RETAILER LOCATED IN THE CITY OF COUNTRYSIDE, ILLINOIS. ALL ENTRIES MUST RECEIVED BY 1/31/09. 1.HOW TO ENTER: Complete an Official Entry Form attached to the original receipt from place of purchase - Official Entry Forms are available at the City of Countryside web site: http://www.gocountryside.net or at City of Countryside City Hall at 5550 East Avenue, Countryside, Illinois 60525. SELECTION OF WINNER: Winner will be determined from all eligible entries received in a random drawing to be conducted on or about 2/11/09 by the current Mayor of the City of Countryside at the regularly scheduled City Council Meeting, whose decisions are final on all matters relating to this Sweepstakes. Winner will be notified by sending him or her one email at the address indicated on the entry form (or one telephone call/message at the number indicated on the entry form) on February 12, 2009, with one follow-up telephone call/message on February 19, 2009, and a second follow-up on February 26, 2009, at the telephone numbers indicated on the entry form. 4.PRIZES AND ODDS: One (1) Grand Prize equaling $5,000 to be awarded as a single or up to five (5) different gift card(s), from a City of Countryside Merchant(s), to the winner. Odds of winning depend upon the number of eligible entries received. Sponsor reserves right to substitute prize of equal or greater value. No other substitution or transfer of prize permitted. Sponsor responsible only for prize delivery; not responsible for prize utility, quality or otherwise. Taxes and fees, if any, are the sole responsibility of Winner. 5.ELIGIBILITY: Sweepstakes open to individual legal residents of the 50 United States and the District of Columbia, age 18 or older as of 11/1/08, except employees (and their immediate families and members of the same household which include spouses, parents, children, brothers and sisters as well as adopted and step members) of City of Countryside. 6.VERIFICATION OF WINNER AND DELIVERY OF PRIZES: Winner will be required to sign an affidavit of eligibility and liability release . Prize may be awarded to alternate Winner if affidavits/releases are not returned within 14 days after mailing to Winner or if prize notification or prize is returned as undeliverable. Winner agrees to the use of name, address, likeness, and/or prize information for promotional purposes in any medium without additional compensation to the extent permitted by law. 7.GENERAL: This Sweepstakes is void outside the United States and where prohibited or restricted by law and is subject to all applicable federal, state and local laws and regulations. This Sweepstakes will be governed by the internal laws of the State of Illinois. Any and all legal actions or claims arising in connection with this Sweepstakes must be brought in a court of competent jurisdiction within the United States of America. Sponsor not responsible for late, lost, damaged, incomplete, misaddressed, postage-due, stolen, misdirected or misplaced entries, mail or communications. Sponsor reserves the right, at its sole discretion, to disqualify any entrant (and all of his or her Sweepstakes entries) from this Sweepstakes or any other promotion conducted now or in the future by Sponsor or any of its affiliates if he/she tampers with the entry process or if his/her fraud or misconduct affects the integrity of the Sweepstakes.
